Four people have died and one child remains in critical condition after an apparent carbon monoxide poisoning inside a vehicle in Toledo, Ohio, on Wednesday, July 15, 2026.
Family Found Unconscious Inside SUV
According to Toledo officials, a grandmother was driving her four grandchildren when their Ford Explorer experienced a flat tire. She pulled into a parking lot in the 300 block of Phillips Avenue.
While waiting for assistance, the family remained inside the SUV with the windows closed and the air conditioner running.
The children's father later arrived to help and found all five occupants unconscious inside the vehicle at about 11:10 a.m. Emergency crews responded and transported everyone to nearby hospitals.
Four Victims Have Died
Authorities confirmed that the grandmother and three of her grandchildren have died.
The remaining child remains hospitalized in critical condition.
Investigation Points to Carbon Monoxide
Toledo Fire Chief Allison Armstrong said investigators tested the vehicle and detected dangerously high levels of carbon monoxide inside the passenger compartment after starting the engine.
Officials said there is no indication of foul play.
The Ford Explorer has been impounded as investigators continue examining the vehicle. Authorities believe an exhaust leak may have allowed carbon monoxide to enter the cabin through the ventilation system.
